26/27 School Year Sports Permission Slip - Grade 4-6 ONLY - Please read entire message.

WILL BE AVAILABLE ON AUGUST 12TH!!

The sports program at Fugman is for grades 4th-6th ONLY. Please mark EVERY sport your child will be doing this year. If you are unsure please mark the ALL! They are not required to do them just because you mark them but you can only fill the permission slip out ONE time per year.
