Click here to access the forms. You will need your COVID Vaccine Card and a TB test/assessment to complete the online form. The Volunteer Form and TB Assessments are valid for 4 years. Remember you must check into the office with your ID
every time you volunteer. Please be sure you have communicated with your child's teacher to see if they are accepting helpers (an email is suggested). You will hear from your child's teacher once you are cleared to volunteer.
